Montgomery County Suicide & Overdose Fatality Review Team: 2025 Community Annual Report

Did you know that Montgomery County has a Suicide & Overdose Fatality Review (SOFR) Team? This team works to prevent suicide and overdose related deaths by reviewing confidential death reviews, honoring the stories and lives of those we have lost, and identifying opportunities to improve systems, services, and programs.

This team is made up of over 20 community agencies who are dedicated to finding missed opportunities for intervention and preventing future deaths.

This year, SOFR has created its first Community Annual Report for the 2025 year!
